In my experience, always standing in front of the doctor, with him (have not had a female doctor) seated. As many others have said, hernias are more apparent (I have had surgeries on both sides for this), and many qualities of the scrotum & testicles are more apparent, such as difference in size, depth of "dangle", reaction of the dartos muscle, condition of the spermatic cords (tangles, cysts, swelling, torsion etc.). While I can enjoy fantasies about this, the actual experience in the doctor's office is devoid of any component of arousal. I can also imagine how turned on the doctor must be after a constant parade of patients, day after day, year after year. Within our gender, and aside from serious abnormalities, whether congenital or acquired, we're all the same. I have often wondered, "What's the big deal?"
