I'm a 63 y/o man, and once at about 22 I saw a urologist in my hometown for leakage. He did a very thorough (and high pressure) digital prostate massage to not only examine me but obtain prostate fluid for analysis. He had me hold a glass microscope slide near my penis tip and kept massaging until I could feel the prostate fluid flow down my penis and out onto the slide. There was no orgasmic sensation or ejaculation, just physical milking of the gland until it exuded the fluid and flowed out. Now I recently began trying prostate massage myself, and it's physically the same, except the urologist pushed MUCH harder to get the fluid so that it was borderline painful (yes, I remember it vividly 41 years later!). When I do it, it's a good deal of pressure with a stainless steel toy, but produces extremely pleasurable sensations just like having a small orgasm, and the fluid gets pushed along I think more because of repeated prostate (or nearby prostate-related muscle) contractions. It turned out I had bacterial prostatitis, nicely treated with an older penicillin derivative (carbenicillin) available in 1974. Trust me, guys, your routine digital prostate exam checking for size and texture is nothing compared to your doc trying to get prostate fluid out of you. So do whatever you can to keep your prostate healthy and avoid this particular medical "pleasure"! My understanding is that lots of orgasms (any way you do them) is potentially helpful.
Oh, and I'd be delighted if my next urologist was a woman!
