So I had a physical with a new doctor today. My last doctor ran a quick in quick out pill mill practice. His care really lacked the personal touch and he didn't really seem concerned. He was around 45 or so and the doctor I have now is 41.

I was led into the exam room by a black MA with a disconnected demeanor. She took the vitals and left without giving me a gown. Didn't know if that was going to be a good or bad sign. A few minutes later the doctor entered the room and took a history. He was dressed in a light blue shirt like on the picture with nice textured khackis and dark dress shoes. Ran through the usual battery of questions asking about prior surgeries, family history, medications, drugs or alcohol, medical conditions. I told him of hemorrhoid problems that my last doctor hadn't even bothered to look at. After the interview he started the exam.
He started with the ear exam by pulling my ear up out and back then sliding the otoscope in. He repeated the exam on both sides and proceeded to do an eye exam. This involved looking at his nose and testing the peripherals and following his finger to test eye muscles. Next he shined the pen light in my eyes to check pupil reflexes. The next exam was of the throat and tongue using the tongue depressor. the doctor examined the thyroid gland on my neck and then moved on to checking my arms. He put two fingers on the brachioradialis and tapped his fingers with another two to try to elicit a reflex on both sides. Checked my arms for any moles which might be precancerous.
I was then instructed to remove my shirt so he could listen to my heart sounds. I unbuttoned my shirt and placed it on the chair by the exam table. He placed that cold stethoscope onto my chest near the sternum. He then moved it to the right, to the left, and under the nipple. After completing the CV exam he positioned himself behind be on the exam table leaning against it. He had me take deep breaths in to listen to my breathing sounds commenting that everything was normal (thank god, because I had been smoking heavily). He started using two thumbs and palpating my upper vertebrae of the spine before coming across a mole he was mildly concerned about. I am fair skin blonde hair blue eyes so skin cancer is fairly common in my family. Glad this doctor actually cared, the last one would never have caught that and a third year med student would do a better exam.
The doctor then extended the exam table and told me to lay back. I lowered myself with the crinkle of the paper on the exam table. He placed the stethoscope against my abdomen and it made me jump a little bit because it was cold. It made me involuntarily twitch my muscles. He listened to some digestive sounds and then had me sit back up. The doctor then went on to say that if I had any concerns with the testicles he would do the exam, but it wasn't entirely necessary. I told him that I wanted it checked just in case and with that he was on the stool slipping on the white latex gloves. He instructed me to hop off the table and stand in front of him removing my pants and underwear to mid thigh level. Going on to say that some patients are bashful and don't want that part of the exam. I am comfortable with my size in general so I don't realy care who sees it. At first I didn't want to look down because I thought I might pop an erection if I did but he didn't look up at all so I looked down. He worked intently sitting on the stool rolling each testicle between his thumb and first 2 fingers. The exam didn't take long and I had enough self control to not get an erection.
He removed his gloves so I started to put my pants back on but he stopped me. I was instructed to bend over the exam table and place my elbows down on the table. I heard the drawer open and the doctor took out a paper package and a tube of lubricant. He spread my cheeks with one hand and palpated around the area looking for skin tags. The doctor then placed a glob of lubricant on his finger and inserted it shallowly asking if I felt any pain. I said no and he told me sorry I have to do this but I am going to examine your prostate. You should feel some pressure like you're going to have a bowel movement but you wont feel any pain. He placed his finger in lightly to spread some lube and allow the sphincter to relax. There was a pause which seemed to take a while, I think he was opening a hemo occult card to test for blood. Then he slowly slid his finger in knuckle deep and I realized his big his fingers were. Not like meaty fat fingers just kind of wide and pretty long. At first it was mildly uncomfortable but then he reached the prostate and started prodding. I got this tingle in my urethra that made me go nearly full mass and I would prob have a throbbing hard on if he had stayed in there another 20 seconds. He gave me a tissue to remove the lubricant and then I quickly put my pants and underwear back on to allow my erection to go back down.I am sure he saw it but I didn't say anything I am sure its a pretty normal reaction fetish or not.
He then gave me three options of checking internal hemorrhoids. One would be to skip it, the second would be to do an anoscope and if he caught anything he would refer me to a specialist, then the third was to just refer me to a specialist. I chose option 2 of course and went with the anoscope. He had me lay in Simm's position on the exam table and draped me leaving the room to grab the anoscope. When he returned he drenched it in lubricant and went behind me telling me it was going to be uncomfortable. He spread my cheeks really wide and slowly inserted it. This one gave a good amount of pressure which I sort of enjoyed and he pushed it in rather slow but didn't go all the way. He had it in there for a good minute and slowly spun it out to where I could hardly feel it coming out. He gave me some more tissues and left the room again to allow me to change.
Overall I am happy with this physician because he seemed really knowledgeable and actually cared about his patients. He also was not afraid to do some of the more hairy and personal tests. It is exciting to think about now but def not the reason why I went in the first place.
