The next visit was a month or so later. This time, I had to see the physician's assistant (male again, unfortunately). I ended up having my blood pressure taken again and then was led down to a different exam room this time. The nurse asked for another urine sample so I went across the hall and was able to produce another specimen. After that, she came back and told me that the P.A. would be in shortly to talk to me. He came in after a few minutes and told me that we would be doing a 3-visit process and that I would be using a compound drug called tri-mix. It is an injection into the base of the penis that expands the blood vessels and produces an erection. He said it's like Caverject, only less painful. At first, I cringed at the thought of jamming a needle into my genitals, but I knew that something had to be done. He did tell me that the base of the penis is not that sensitive and that there aren't that many nerve endings there, so the shots are really not that painful. He explained that I would have to be examined again (by him), and then he would do an injection and examine me again after I became erect to check how the drug was working for me. The next 2 visits would determine how much tri-mix was right for me, depending on how long it took for my erection to go down. Supposedly, the erection isn't supposed to go beyond 4 hours---if it does, I have to get to an emergency room--- pronto. After the discussion, he left the room and the nurse showed up again. She told me to undress from the waist down and to cover myself with the towel that she handed to me. She left the room and I took off my slacks, shoes, socks and briefs then sat on the table with the towel over my lap. He came back in a few minutes later with a syringe and a small vial. He to me that he would be examining my penis, rectum, doing a prostate exam and also what he called a bulbocavernosus reflex test---which he said is a nerve reflex test that evaluates nerve sensation in the penis. I had NO idea how that test was done and it was definitely different. He asked me some questions--a lot which were the same as the urologist asked me. Then, he told me to take my shirt off (I had a t-shirt on underneath). He listened to my heart for quite awhile---front and back. Then, he asked me to stand in front of him. I put the towel aside and stood up. He put his gloves on and took a few minutes to examine my penis--he did a lot of the same things that the urologist did---checked the shaft, the head and spread the urethral opening apart. He did a testicle exam and had me cough to check for a hernia. He spent a few seconds just looking at it, which was an incredible turn-on for me. I could feel myself getting aroused--after all, the only thing I had on was my t-shirt. He asked me more questions about attaining an erection, how long I was able to maintain it before losing it. He then asked me to lie down on the table and that he was going to do the rectal exam and the reflex test. After I had laid down, he told me to grasp the back of my thighs and to pull my legs to my chest. He explained that he would grasp the head of my penis and that he would squeeze it gently---and that there should be a rectal contraction which somehow is supposed to mean that the nerve sensation in my penis is normal. He said that if it’s abnormal, that could explain part of my erection issues. I pulled my legs back to my chest and he grabbed my penis. He squeezed and I did feel my rectum contract—it was kind of an involuntary reaction I guess. At any rate, he did say that he saw my anus contract and that I was normal. After that, I felt him poke around my rectal area and he asked about hemorrhoids, etc. He asked me to strain down, then he told me to stand up and bend over the table for the prostate exam. I bent over, then was asked once again to bend at the knees. He inserted his finger and did the prostate exam and he said that everything seemed normal. After that, I was able to sit back down on the table again and covered up with the towel. He told me that everything seemed normal. I basically got the tone from him that he thought it was all in my head with the erection issues and all. But, he agreed to try the tri-mix injection anyway if I wanted to go ahead with it. I told him that I did, so he said he would search for a good area on my penis to do the injection. The injection can’t be directly into a vein, so he had to examine me once again. He did this exam while I was still sitting, but I had to move forward on the table and place my feet on a stool with my legs apart. It was pretty damned intense while I watched him handle my penis, looking for a spot to do the injection. He pointed to the right side of the shaft, telling me that’s where I should be doing the injection, but to do a different spot each time. He took a alcohol swab and cleaned the area where he was going to do the injection, then took the syringe in his hand. He grasped my penis by the head and pulled straight out, instructing me the entire time on what he was doing. I was really getting nervous now, since I was expecting extreme pain when he did the shot, but it really wasn’t that bad. The initial penetration was a sting, but after that, it wasn’t bad. It took him 3 or 4 seconds to do the shot, and that was it. He told me to massage the penis and to work the medicine in, so I felt like I was masturbating in front of the guy. Talk about a turn-on…holy cow. After that, he told me that it should take about an hour for the tri-mix to work, and that I should be fully erect. He gave me some magazines to look at, and left the room. He told me that I couldn’t go home until the erection was fully gone and that he would be back to periodically check on me. Well, I ended up waiting 4 hours for the erection to subside. He came back after about 45 minutes, and by then I was fully erect. That was unbelievable---to be fully erect in front of a P.A.. I’ve never been through anything like that in my life and probably never will again.
