Since you asked...
For me, it starts with what I wear - lol. I prefer shirt and tie...I think it sets the "Doctor-Patient"tone. Then I ask questions...lots of them. very personal, of course, insisting on detailed responses. Then a very in depth exam, starting with the face,neck, and extremities. It may take a good half hour or more to get to this point. After all, a thorough exam is what is needed. Then I move on to a more personal exam, beginning with the back, shoulders, and joints, only removing articles of clothing as necessary. Physical responses are tested as we proceed, insuring that there are no problems.
As we proceed, a detailed examination is, of course, required, whcih takes a lengthy amount of time. it entails numerous devices designed to elicit physical responses. however, none of them are painful, as pain is not the desired response. rather we want to insure you are capable of normal physical responses.
It is likely that a thorough cleansing will be needed in order to continue the exam, as we don't want anything in the way of insuring that you are in good health. this is normal, as most people do not clean themselves internally very often, and it should be done regularly. This can be done several ways, but I prefer having my Patient kneel on a bed (a table is too hard and uncomfortable), with her head down, legs apart. this allows easier access to assist her and insure that it is administered properly. Several cleansings are usually required if she has not been cleaned recently. In most cases, a small (1 qt.) injection is used initially, as it is more easily accomodated, with larger volumes to follow. I always assist my patients in retaining it for a short period, as they seem to appreciate the personal ministrations.
Frequently, this is accompanied by arousal on the part of the patient. This is to be expected, and is encouraged, as it is a sign that your ability to respond to personal touch and stimulation is healthy. Indeed, this is always encouraged and aided, invariably to completion. The means of completing this vary from patient to patient, and can include anything from manual to oral stimulation up to penetration during the procedure.
Lastly, you asked how I receive my pleasure through thsi process. There is an intense satisfaction in giving the exam, especially when the patient experiences her own personal release. As for my physical satisfaction, that is something to be determined between me and the patient. Limits need to be respected, no matter what.
